Located near Upper Fairmount on Maddox Island. The cemetery was described as about 50 feet by 50 feet, with a number of old trees, about halfway up the lane to the old Capt. Ford house off Fishing Island Road.

The contributors placed the site within the ancient patent Inclosure, issued to Thomas Maddox in 1740 for 590 acres as a resurvey of Maddox Inclosure (1680). John Maddox was taxed for 590 acres on the 1783 tax list. The site is also near the home of Capt. William T. Ford shown on the 1877 Atlas.

The survey recorded three table-top graves, two other stones, and several field stones apparently marking the cemetery boundaries.
